• disegni o fotografie del design (anche campioni, se sono modelli bidimensionali) ed eventuale descrizione
• nome e dati anagrafici e fiscali del richiedente il design.

Documenti ulteriori da produrre anche dopo il deposito della domanda

• lettera di incarico per la rappresentanza del richiedente di fronte all’Ufficio Italiano Brevetti e Marchi.
